INTELLECTUALLY GIFTED REFERRAL TO PLACEMENT PROCESS

 

***REFERRAL***

 

A student may be referred at any time by anyone.

 

Any question concerning hearing and/or vision problems must be cleared before proceeding.  If there is a concern, please ask the school nurse to conduct a hearing and/or vision screening.

 

REFERRAL FORM (Attachment A):

Part I & Part III – the classroom teacher will complete every space

Part II – the CREATE teacher will complete this part - must show documentation of two or more of the following and documentation must be attached to the referral form

1.      results of group and/or individual intelligence test(s) administered

within the last twelve months

2.      grades from the current or past school years (must be passing grades)

3.      demonstrated exceptional individual achievement in academics,

leadership, and/or creativity such as recognition of abilities at a state,

regional, or national level, or

4.      results of group and/or individual achievement test(s) (individual

achievement tests must have been administered within the last

twelve months)

 

NOTE: During the referral process, any individual intelligence and/or achievement measures utilized for the referral should have been administered previously, unless written parental permission is obtained prior to administering such test(s).

 

***LOCAL SURVEY COMMITTEE REVIEW***

 

Forward the referral form (attachment A) (with attachments) to the school level Local Survey Committee (LSC) chairperson (the CREATE teacher).  Documentation of LSC meetings must be kept by the chairperson.  The LSC members are the CREATE teacher, a regular education teacher, and the school administrator.

The school level LSC decides whether the information meets the criteria required (see attachment F) to enter the assessment phase.  The school level LSC decision is documented on the Local Survey Committee Report form (form GE-1).

                                                                                                                                   

***PARENTAL PERMISSION FOR ASSESSMENT***

 

Once the school level LSC has made the decision to refer a student for individual testing, the CREATE teacher must obtain written permission for testing on Part II of the Gifted Pupil Personal Data Sheet. (Attachment C) (Complete all information above Part II.)

Invite parents to meet with you by sending the Notice of Meeting/Permission to Assess form (form GE-2) to the parent.

At the meeting, inform the parents of the Family Rights and Privacy Act (form GE-3) and give them a copy.

If the parents want their child assessed for participation in the gifted education program, have them sign Part II of the Gifted Pupil Personal Data Sheet (Attachment C).

Send the entire file to the special education central office to the attention of Teresse Lawson.

A PARENT MAY CHOOSE TO NOT HAVE THE CHILD ASSESSED.  THE PARENT MUST SIGN A REFUSAL OF INITIAL ASSESSMENT FORM (form GE-4).  THE ENTIRE FILE IS GIVEN TO THE SCHOOL PRINCIPAL.

 

***ASSESSMENT ***

 

The CREATE teacher will give checklists to the teacher and all documentation must be attached to form GE-5a-b.

At least three of the measures outlined below must be collected/administered: (Document on form GE-5a-b.)

1.      norm-referenced group intelligence test (administered within the last twelve

months) with a minimal score of 90th percentile,

2.      published checklists of characteristics of gifted students with a score in at

least the high average range,

3.      norm-referenced creativity test(s) with a minimal score at or above the superior range, or published test(s)/checklist(s) with a score in at least the high

average range,

4.      published test(s)/checklist(s) of leadership abilities with a minimal score in at

least the high average range,

5.      exceptional individual accomplishments in an academic, leadership, and/or

creativity area such as recognition at a state, regional, or national level for

outstanding work accomplished in at least one of the areas specified, or

6.      norm-referenced group or individual achievement test(s) (individual test

must have been administered within the last twelve months) utilizing scores

in reading comprehension, language arts, science, social studies, and/or mathematical reasoning with a minimal criteria of 90th percentile.

                                   

 

***ASSESSMENT REPORT***

 

NOTE: If any of the measures were included on the referral form, the results may be used as a part of the assessment if the timeline and percentile criteria are met. 

 

If referral and assessment criteria are met, the file is given to an examiner for the final individual intelligence assessment.  After assessment, a report is written and the central office LSC determines eligibility.

 

The examiner will complete the IQ section on page 2 of form GE-5a-b.

 

The examiner will send two copies of the Gifted Pupil Personal Data Sheet (Attachment C) and two copies of the report (in a blue folder) to the CREATE teacher.

 

The examiner will give a copy of the Gifted Pupil Personal Data Sheet (Attachment C), a copy of the report, and the remainder of the file to Teresse Lawson.                                   

 

***PLACEMENT DECISION***

 

The CREATE teacher will send a Notice of Meeting/Assessment Report form (form GE-6) to the parents.  Document when it was sent.  The teacher will meet with the parents to go over the report and get the Permission to Place form (form GE-7) signed, if applicable.

 

The CREATE teacher will send a copy of the Permission to Place form (form GE-7) to the special education central office to the attention of Teresse Lawson.

 

The teacher will explain the district Instructional Management Plan and the CREATE program.

 

The parents will be given a copy of the Gifted Pupil Personal Data Sheet (Attachment C) and the assessment report. The student may begin CREATE classes if eligible.

 

***REFUSAL OF PARTICIPATION***

 

A PARENT MAY REFUSE FOR THE STUDENT TO PARTICIPATE IN THE CREATE PROGRAM.  THE PARENT MUST SIGN THE BOTTOM PORTION OF THE PERMISSION TO PLACE FORM (FORM GE-7).  THE ENTIRE FILE MUST BE SENT TO THE SPECIAL EDUCATION CENTRAL OFFICE TO THE ATTENTION OF TERESSE LAWSON.

***NOT ELIGIBLE***

                                                                                                                                   

IF THE STUDENT DID NOT QUALIFY FOR PARTICIPATION IN THE CREATE PROGRAM, GIVE THE PARENTS A COPY OF THE GIFTED PUPIL PERSONAL DATE SHEET (ATTACHMENT C) AND REPORT.  SEND THE ENTIRE FILE TO THE SPECIAL EDUCATION CENTRAL OFFICE TO THE ATTENTION OF TERESSE LAWSON.

 

***ANNUAL REASSESSMENT***

 

At the end of each year, the CREATE teacher will conduct an annual reassessment.  A committee comprised of the CREATE teacher, a regular education teacher, and a school based administrator will document the meeting and decision on the Gifted Education Annual Reassessment form (form GE-8).  File the form in the student’s CREATE folder.  If there is a decision that the student should not continue in the program, the parents must be notified and given an opportunity to discuss the decision.  The district CREATE coordinator must be invited to this meeting.

 

NOTE: If there is a decision by the teacher, administrator, and parents that the student should discontinue CREATE classes at any time during the school year or during the annual reassessment the parents must sign the Discontinuation of Services form (form GE-9).  The file must be sent to the special education central office to the attention of Teresse Lawson.

The student may enter the program again at a later date if the parents sign another Permission to Place form (form GE-7).

 

***OUT OF DISTRICT STUDENTS***

 

If a student moves to our district from another Mississippi school district and has a gifted eligibility from that district, the student may be served in the CREATE program. The parents must sign a Permission to Place form (form GE-7).  The CREATE teacher will send a copy of form GE-7, the student’s name, the date of birth, and information about the previous school to the special education central office to the attention of Teresse Lawson.  This district must request the records from the previous district.

 

***OUT OF STATE STUDENTS***

 

If a student moves to our district from out of state, the student may not receive services in the CREATE program until there is a Mississippi eligibility.  Please complete this referral to placement process for that student.
